Have you tried things that are more foreplay related rather than just actual sex? Perhaps amping up foreplay will get him more turned on so that he will remain hard for sex. My boyfriend doesn't have a foot fetish, but we do use a position that might make it easier for him to be in contact with your feet.
Kneel on the edge of the bed, with you legs hanging off a little, and bend the top half of your body down, so you're sort of lying across the bed with your rump in the air (it helps to have a pillow for your head).
